Your child's school (as well as Lumen Learning Trust) sometimes takes photographs and videos of pupils. We also sometimes publish pupil names. We use images or names for a variety of reasons and their use is an invaluable tool in helping us to celebrate pupil educational journeys and communicating the messages of your child’s school and the Trust.
For any images displayed externally (categories 2-5) the school will not feature the child's name in any format, unless you have specifically consented to it.
For any images displayed internally (category 1) any names featured will be the first name and initial of surname by default.
By choosing to tick either the Yes or No boxes below, you are confirming whether you give consent for an image to be captured and used in the following circumstances:

Declaration
• I have read and understood the consent asked of me above.
• I understand that I will be given the opportunity to review my consent choices at the end of Year 2.
• I understand that I may change my consent status at any time by notifying the school in writing to office.ewellgrove@lumenlearningtrust.co.uk or Office Manager, Ewell Grove Primary & Nursery School, 29 West Street, Ewell, Surrey, KT17 1UZ.
• Consents given apply for up to a maximum of 5 years after my child has left the school.
•The school will not use the full name of my child in any accompanying text or caption for an image used.
• I promise that if I, or members of my family, take photographs or video recordings at a school event, these will be kept for family use only.
• I, or members of my family, will not publish any material online that features an image of any child other than my own.
